Typical Inclusions – Wound Dressing (Home Care)
1. Pre-Procedure Assessment
Verification of doctor’s prescription
Patient identification and consent
Assessment of wound type (surgical, pressure sore, diabetic, etc.)
Evaluation of wound condition (size, depth, infection signs)
Pain assessment and explanation of procedure
2. Preparation
Strict hand hygiene and aseptic technique
Preparation of sterile dressing tray
Use of appropriate PPE (gloves, mask)
Positioning of patient for comfort and accessibility
3. Wound Cleaning
Removal of old dressing carefully
Inspection of wound for discharge, odor, infection
Cleaning with normal saline/antiseptic solution
Debridement (if minor and within scope)
4. Dressing Application
Application of prescribed ointments/medications
Selection of appropriate dressing material
Sterile covering and securing of dressing
Pressure relief measures (if required)
5. Infection Control
Maintenance of sterile field
Prevention of cross-contamination
Proper handling of wound exudate
6. Post-Procedure Care
Ensuring patient comfort
Pain management support (as per advice)
Guidance on positioning and wound protection
7. Documentation
Recording wound condition and progress

Date, time, and observations

Safe disposal of contaminated materials
